Locum Band 6 Community Psychiatric Nurse to join their Community Team.
Hours: Monday-Friday 9am to 5pm
Job Summary:
This role will involve a number of elements which will support people with mental health conditions in the new integrated mental health teams. These roles will be key in providing professional support to the new primary care network based teams and will provide mental and physical health support helping people to engage with their local communities and benefit from all the supports available locally. The post holder will be part of a multi-disciplinary team which will cover a Primary Care Network or locality and will work across networks to form a truly innovative and ground-breaking support structure.
The objectives of the services are:
To consider an individual’s needs holistically – mental and physical health as well as social needs
To reduce the need for repeated long assessments and story telling
To provide early advice and guidance with signposting to the relevant services where appropriate therefore supporting individuals to make full use of community resources available
To bridge the gaps that can appear between community mental health care and primary care, acute care, social services and support.
To ensure easier access to the right level of care such as group programmes, therapy etc.
For Care plans to be person centred – developed with the individual and others involved in their care
Provide an enhanced level of support to Primary Care colleagues through advice and training to improve skills and capacity in responding to the needs of people with mental health needs.
To work within an agreed process for safe transfer from WLT to GPs as well as and rapid rereferral, when appropriate
That individuals transferred to their GPs under this process have access to appropriate advice, advocacy, vocational support to access social networks
To operate within legislative frameworks – Mental Health Act 2007, The Mental Capacity Act 2005, The Care Act 2014 and other relevant health and social care legislation.
Key Result Areas & Performance:
The Integrated Mental Health Team mental health nurse will have a number of key roles and will undertake the following duties:
Clinical
Provide holistic and specialist bio-psycho-social assessment for anyone experiencing mental health difficulties and offer specialist nursing assessment where required
Provide advice to primary care to help them provide better support to patients and to (where preferable) meet their mental health needs within a primary care setting.
Carry out robust risk assessments, working collaboratively with the patient and their carer to provide a plan that will support them remaining safe.
Provide access to a broad range of specialist psychosocial interventions for individual patients where needed.
Provide expert advice regarding the pharmacological treatment options available to mental health patients to patients and non-mental health professionals, seeking clinical guidance from the designated Consultant Psychiatrist/GP in relation to any specific prescribing advice.
Where required, nurses working within the team will administer intra muscular medication to patients for whom this is prescribed, assessing the effects and side effects at each appointment, working within the NMC guidelines and the Trust’s medicines policies
To maintain and the safe handling and storage of medication at all times, ensuring the safe disposal of clinical waste and all sharps.
To ensure infection control measures are strictly adhered to both on site and when visiting off site locations.
To support patients to identify and optimise their physical health requirements through assessment, intervention and signposting.
Be required to work in conjunction with individual members of the primary care team, patients and carers to develop personalised planning for future wellbeing.
To promote and empower patients to reach their highest level of recovery and functionality using a strengths based approach
Develop systems of engagement with key 3rd sector partners to identify and promote access to mainstream activities.
To act within the NMC code of conduct
To act as practice assessors and supervisors to student nurses on placement with the team
Demonstrate a high standard of record keeping and documentation with adherence to Trust policy and an understanding of NMC professional guidelines and application of confidentiality.
To understand their duty of candour in maintaining an honest and open relationship with the patient and their loved ones.
Work with colleagues to develop and promote the service philosophy, framework of care delivery and strategic objectives, taking responsibility for specific areas of policy and strategy delivery.
Work with colleagues to develop clinical expertise and high standards of professional practice.
Deliver care that is evidence based, reflecting current best practice and research (including NICE guidelines), demonstrating clinical excellence and high standards of professional practice to other staff.
Demonstrate sound understanding of the legal and ethical issues in caring for people with a mental health condition. In particular, demonstrating sound understanding of the Mental Health Act (1983) and Mental Capacity Act and its use in a non-mental health setting.
The post holder, in carrying out the above clinical work will identify the themes/training needs that emerge within the wider primary care team. The post holder will work with others as part of the project to put together training packages, and support the delivery of training.
To consider becoming an Approved Mental Health Act Professional as in the MHA
