Our clients are urgently seeking to recruit a
Locum Band 6 Community Psychiatric Nurse to join their Recovery Team.
Hours: Monday-Friday 9am to 5pm

Job Summary
The appointee will work in the Community Team.
1. To get to know all patients on the teams caseload, and to review those on their personal caseload at the frequency agreed by the team.
2. To plan, implement and evaluate nursing interventions delivered in the community, and participating in multidisciplinary care plans.
3. To keep in contact with patients’ relatives, friends, carers and professionals in other agencies as appropriate.
4. To administrate, order and maintain a stock of medication and other medical sundries, in accordance with relevant policies and procedures.
5. To co-ordinate the care of those patients for whom the CPN is the nominated care coordinator.
6. To act as social supervisor for those patients for whom the CPN is the nominated social supervisor, including producing pro-forma reports for the Ministry of Justice(after receiving appropriate training, if necessary).
7. To respond, sometimes at short notice, to requests to see the teams patients either individually or jointly with other team members (e.g .when a patient is in the red zone).
8. To attend and participate in all daily team zoning meetings unless absent by prior arrangement.
9. To attend CPA meetings or Tribunals where appropriate.

10.To occasionally participate in scheduled and emergency meetings held under the Multi-Agency Public Protection Arrangements.
11.To be contactable by mobile telephone or other agreed method at all times during working hours.
12.Toassistintheallocationofworkinordertocoverholidays,sickness,absencesandvariable service demands, as necessary.
13.To keep good quality and up-to-date records of scheduled and unscheduled work and notes of assessments and reviews etc, using the RiO computer system or prescription charts as appropriate.
14.To provide teaching and training to student nurses allocated to the Community Team.
15.To comply with all relevant Trust policies and procedures and the Nursing and Midwifery Council Code of Professional Conduct, in particular those policies concerned with the safety of the CPN themselves, their colleagues and patients, and the public.
16.To participate in regular (at least monthly) professional supervision and regular (at least annual) appraisal, including keeping abreast of educational, training and personal development needs
