Our clients are urgently seeking to recruit a
Locum Band 6 Community Psychiatric Nurse to join their Rehabilitation Team.
Hours: Monday-Friday 9am to 5pm
Job Summary
Work closely with the two trusts inpatient rehabilitation units, as well as inpatient rehabilitation units in the and out of area.
Work closely with housing providers and other agencies to ensure appropriate care plans are in place promoting independence, recovery, and rehabilitation.
Lead on caseload management of clients in inpatient rehabilitation units.
Facilitate client step downs from hospital to supported accommodation placements in collaboration with social care.
Contribute to more effective systems for future planning and reviewing of clients in rehabilitation placements.
Ensure high quality care is evident in referrals to the Single Point of Access (SPA).
Work with the Clinical Assessment Team.
Support existing providers.
Support the flow of care throughout the inpatient and community pathways and reduce delays in discharge.
Maximise community stakeholders, including the voluntary sector, to enable full recovery and independence for our clients.
Provide support and guidance to community teams, supported housing providers and key partner agencies.
Assist with reducing the need for out of area placements and focus on supporting people locally in the area.
