Our clients are urgently seeking to recruit a
Locum Band 7 Occupational Therapist to join their Primary Care Liaison Team.
Hours: Monday-Friday 9am to 5pm
Job Summary:
The service is committed to work in partnership with service users and their carers, as well as with other statutory and non-statutory services. Our aim is to provide strengths based and recovery orientated interventions in order to maximize the quality of life and wellbeing of service users. The post holder will work exclusively within the Primary Care Liaison Team (PCL)
The PCL team is one part of the Wellbeing Service, working alongside the psychological therapies team, provided within the IAPT Talking Therapies framework. The PCL team operates through a single point of referral and the service model comprises four main elements:
1) Diagnostic and Review Assessment;
2) Brief follow up periods and interventions
3) Providing consultation for GP’s, including clinical advice and guidance;
4) Facilitating teaching sessions for internal and external stake holders.
In line with exciting new developments in the NHS, the creation of Integrated Care Systems and the formation of Primary Care Networks to deliver ‘neighbourhood’ based care, the PCL team has expanded. This has meant a group of GP based B5 mental health support workers have been aligned with the PCL team. A key part of this role is to act as a clinical and managerial supervisor for these roles. The Primary Care Liaison team also provides effective liaison and transfer of care between primary and secondary care mental health services. This integrated service model facilitates a range of ways of working from booking service users directly into the Talking Therapies Program to offering mutual learning opportunities for staff across psychological and psychiatric aspects of care. The overall goal is that service users receive a timely, effective, and integrated Primary Care Mental Health intervention and support service users with the transition into other mental health services if clinically indicated.
The post holder will promote clinical excellence and expertise in providing care for patients with mild to moderate mental health difficulties and support the delivery of an agreed and coherent, collaborative and co-produced care plan, in keeping with care pathways, operational policies and NICE guidelines. The post holder will deliver services to maximise engagement of service users and support them in achieving their recovery goals. The post holder will be involved in service development and implementation of the requirements of the NHS Long Term Plan and the National Service Framework for Mental Health Standard 2.
Key Responsibilities:
Clinical
1. To promote clinical excellence and work within the framework of evidence based medicine and best practice.
2. To liaise effectively and pro-actively with GP Practices, Integrated Recovery Hubs/Single Point of Access (SPA), and other voluntary organisations, creating positive working relationships and positive service experience for services user, carers and other professionals.
3. To encourage and support service users’ and carers’ involvement in their care and treatment.
4. To work as an effective member of the multi-disciplinary team ensuring effective communication with other team members, other professionals and members of the community.
5. To ensure service users receive a timely and effective Primary Care Mental Health intervention, as agreed by pathways, protocols and NICE guidelines.
6. To ensure referrals are processed appropriately.
7. To deliver comprehensive biopsychosocial assessments, diagnostic formulations and clinical reviews and clinical interventions with and for service users
8. Offering medication reviews and advise to patients, carers and GP’s
9. To ensure effective transfer of secondary care service users to Primary care, including service users on depot medication (Depot Enhanced Service). This transfer will be based on detailed clinical reviews and systematic analysis of patients’ needs which will be undertaken by the post holder.
10. To support GPs and other professionals in primary care with assessing and treating patients by role modelling, facilitating, and training.
11. To deliver consultation, advice and clinical supervision to other members of the clinical team regarding Primary Care service provision.
12. To lead multi-disciplinary and multi-agency clinical meetings when required.
